Acupuncture for Stroke Recovery: What It Can and Cannot Do

Malaysia's Ministry of Health offers acupuncture to stroke patients. What the research says, when to go to hospital instead, and what a course looks like at our Setapak clinic.

By Dr. Jaron Yeong

Acupuncture can help some people with movement, pain and daily function after a stroke, but it works alongside hospital care and rehabilitation, not instead of them. Malaysia's Ministry of Health already offers acupuncture for post stroke patients in its integrated hospitals, so this is not a fringe idea here. It is also not a cure, and anyone who promises one is overselling.

If the stroke is happening now, go to hospital

Sudden face drooping, weakness in one arm or leg, or slurred speech: call 999 and go to the nearest emergency department. The treatment that limits brain damage happens in the first hours, and it happens in hospital. Do not come to a TCM clinic first, including ours.

We start once the patient is medically stable and their hospital doctor is happy for them to begin rehabilitation. If they take blood thinners such as warfarin or aspirin, tell us at the first visit, because it changes how we needle.

What the research says

The short answer is promising, not proven. A 2018 Cochrane review of acupuncture for acute stroke found that patients given acupuncture appeared to do better, but mostly in trials where nobody received a sham treatment for comparison, which makes the result less reliable. The side effects it reported were minor.

On safety, the US National Center for Complementary and Integrative Health notes that acupuncture has few complications when done by a trained practitioner using sterile needles (NCCIH). That is the standard we hold ourselves to.

What we work on after a stroke

Depending on the patient, the focus might be a weak or stiff arm and leg, a hand that will not open, a drooping face, shoulder pain on the affected side, constipation, poor sleep or low mood. We start with what the patient and family say matters most in daily life.

Acupuncture can be combined with tuina and simple exercises the family can help with at home. That follows Dr. Jaron Yeong's view on recovery in general: needles release the tissue, and conditioning rebuilds how the muscles work together. Physiotherapy and speech therapy from the hospital should carry on. We never ask anyone to stop them.

What a course of treatment looks like

Stroke recovery is slow, so expect a series of sessions over weeks, not one visit. At the first session the practitioner takes a full history, goes through the medication list, and agrees goals with the family. After a few sessions we review what has changed and decide together whether to continue. If there is no change, the practitioner will tell you so plainly rather than keep booking sessions.

Getting to our Setapak clinic

Jaron TCM is on Jalan Genting Kelang in Setapak, Kuala Lumpur, open every day from 9 AM to 6 PM except Chinese New Year. Family members can WhatsApp us first with the patient's situation to ask whether they are ready to start. Frequently Asked Questions

How soon after a stroke can acupuncture start?

Once the patient is medically stable and their hospital doctor agrees they can begin rehabilitation. We do not treat during the emergency phase.

Does acupuncture replace physiotherapy?

No. Acupuncture is an addition, not a substitute. Keep going with the physiotherapy and speech therapy your hospital arranges.

Is acupuncture safe for someone on blood thinners?

It can be done with care. Tell the practitioner exactly which medicines the patient takes before the first session so the needling can be adjusted.
